In this short course, we will watch a video of an incident and then analyse it to see what went wrong and what could have been done to prevent this incident.

This video will take us through an incident based on the experiences of the UK P&I club. This incident happened on a bulk carrier, during a third-party survey, where the surveyor made a request to test the emergency fire pump which was located in a recessed well, in the steering gear compartment.

Accidents like this can and do happen! Ask yourself… Could this happen on my ship?
